MDS Coordinator - LVN (Healthcare)



The MDS Coordinator (LVN) is responsible for managing the resident assessment process to ensure accurate, timely, and comprehensive completion of the Minimum Data Set (MDS) and all related documentation. This role involves coordinating care plans, overseeing reimbursement schedules for Medicare and Medicaid, and providing administrative and clinical support to the nursing team. The MDS Coordinator ensures the facility remains compliant with all federal and state regulations.

Essential Duties and Responsibilities

Administrative & Coordination

  • Administer patient assessments, oversee the entire assessment process, and set assessment schedules to ensure completion in a timely manner.
  • Coordinate the development, implementation, and evaluation of resident care plans in accordance with regulatory requirements.
  • Create and manage the schedule for all Medicare and Medicaid assessments and ensure accurate documentation for reimbursement.
  • Stay updated on all changes in Medicare and Medicaid coverage and assist in determining the documentation needed for reimbursement.
  • Manage the admission, transfer, and discharge of residents, including preparing and completing all necessary paperwork.
  • Perform routine administrative duties, such as completing medical forms, reports, and evaluations as needed.
  • Review department policies, manuals, and job descriptions and make recommendations for revisions.
  • Participate in facility surveys and inspections conducted by authorized government agencies.
  • Provide data to the Quality Assurance & Assessment Committee as requested.

Charting & Documentation

  • Ensure the accurate and informative charting of nurses' notes that reflect the care provided and the resident's response to it.
  • Receive and transcribe telephone orders from physicians accurately.
  • Complete and file required record-keeping forms upon resident admission, transfer, and discharge.
  • Ensure all documentation for unauthorized discharges is properly entered into the resident's medical record.
  • Maintain the daily census report and submit it to the business office as required.

Clinical & Nursing Functions

  • Provide leadership and direction to nursing assistants and other assigned nursing personnel.
  • Make daily rounds to ensure that personnel are performing work assignments according to acceptable nursing standards.
  • Administer medications and treatments as ordered by a physician, ensuring proper procedures and patient verification.
  • Conduct periodic checks to ensure that prescribed treatments are being administered properly and to evaluate the resident's physical and emotional status.
  • Assist in planning and implementing the nursing services portion of a resident's discharge plan.
  • Collaborate with other resident services to coordinate care and ensure a comprehensive care regimen is maintained.
  • Notify physicians and next of kin of any significant changes in a resident's condition.

Qualifications

  • Education: Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
  • License: Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state
