Community Health Worker (Personal Services)



Fall River Health Services is looking for a Community Health Worker (CHW). In this role you will be responsible for helping patients and their families access and navigate community services, other resources, and to help adopt healthy behaviors. CHW will work with vulnerable populations and or patients experiencing health disparities, ensuring that they have access to care and programs that can help them lead healthier, safer and more secure lives. By helping with the coordination of care, CHW will assist patients with complex conditions and manage office visits, testing services and treatments.

Travel :
Local travel to coordinate with community resourcesLocal travel to meet with patients at community sites such as: food bank, DMV, etc Local travel to attend/present at community outreach events and or promoting FRH in our community

Responsibilities:
Be responsible for establishing trusting relationships with patients and their families, while providing general support and encouragement. Leading support groups, workshops and seminars on topics of interest to the communityLeading and or Coordinating Caregiver education events Assist with Discharge Calls for hospitalReviewrecurrent ED visits, evaluating resource needs for insurance coverage and or primary care provider and applicable resources.Review all self-pay patients and send applicable resources. Work closely with medical providers and other clinical personnel to ensure patients have comprehensive and coordinated care plans. Hold knowledge of community, state, and federal resources and provide referrals for services, etc. to agencies as appropriate.Act as a patient advocate and liaison between the patient/family and community/state/federal service agencies.Promote, maintain, and improve the health of patients and their families by setting patient goals, ensuring patients attend appointments and holding patients accountable.Manage assigned caseload of patients.Maintain HIIPA compliance at all times.Arrange and/or attend staff meetings, training and other meetings as requested.Effectively communicate and demonstrate the mission of Fall River Health Services.Provide social support and informal counseling, advocates for individuals and community health needs, and demonstrate respect by ensuring the dignity of the patient and their family. Frequent contact with other departments such as lab, and x-ray, and the business office.Coordinate information and application for SD Medicaid and other federal/state assistance
The average Salary for this position is $22.18- $26.63an hour, depending on years of experience.
