GENERAL RESPONSIBILITIES & ESSENTIAL JOB FUNCTIONS:
Provide Case Management, CPST, or Crisis service when their service provider is not in the office or otherwise unavailable to provide the services outlined in the Tx Plan.
Collaborate with other CSS staff to determine when they will be out of the office and arrange for their adults in service to be seen.
Explain to any adult why you are providing service for the day
Triage with assigned providers those most in need if the volume exceeds what can be done in a day
Fill in as needed with the Group Program
Provide goal-directed support and solution-focused interventions.
Work hours are flexible to meet the needs of the person served.
Make all person contacts as assigned for the day
The majority of contacts happen out of the office and in community settings.
Weekly contact sheets turned in at the end of the workweek.
Progress notes are completed within 24 hours of service.
Attend tx plan meetings as requested and following a crisis.
EMR scheduler is utilized and kept updated
MINIMUM QUALIFICATIONS:
Age 21+ years old and have a BA/BS degree with an emphasis on Mental Health/human services or equivalent experience.
Excellent interpersonal and teamwork skills
Flexibility, multitasking, high level of organization required and time management skills.
Interest and investment in working with adults who experience Severe and Persistent Mental Illness (SPMI) and their families.
Have completed or will complete any required training within six months of employment.
