Case Manager (Warrick County) (Personal Services)



Join the team at Southwestern Behavioral Healthcare! We're currently seeking a full-time Case Manager to serve our Warrick County office. In this role, you'll be part of a dedicated team providing community-based services including case management, skills training, and therapeutic support to adults - and/or children and families - in an outpatient setting.

WHAT IS THIS POSITION RESPONSIBLE FOR?

  • Conduct case management, skills training & development, and other treatment services according to Southwestern policies.
  • Provide case management services to adults assigned to OP Services including, but not limited to: communicating with all parties involved in the client's treatment; identifying treatment needs and facilitating all necessary referrals; coordinating and attending collateral meetings with all parties involved in the treatment process.
  • Provide case management services to children and families referred by Southwestern Behavioral assessment team and therapists, school, the Department of Child Services, the Office of Juvenile Probation, the System of Care, and other potential referral sources. Case management includes but is not limited to communicating with all parties involved in the individual/family's treatment; identifying treatment needs and facilitating all necessary referrals; coordinating and attending collateral meetings with all parties involved in the treatment process.
  • Provide community-based skills training and development services for adult clients and for children and families. Skills training and development will be determined based on individual/family's identified needs.
  • Provide emergency counseling and crisis intervention when necessary.
  • Maintain appropriate records of services to reflect patient progress and document treatment plans and goals.
  • Maintain documents and submit reports according to established time frames.
  • Communicate professionally, make referrals and coordinate services with community agencies.
  • Become familiar with the Wraparound process of collaboration and service delivery.
  • Must meet efficiency standards which include chart management and patient contact standards.

MINIMUM QUALIFICATIONS:

  • Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice or other related degree required.
  • Experience working with adult individuals who have experienced mental health difficulties is preferable.
  • Candidates must pass required background checks including county/state checks and drug screen.
  • Valid Driver's License.
