Position Summary:
As an RN at Sierra Valley Rehab Center, you will play a key role in supervising staff, coordinating resident care, and ensuring clinical excellence in accordance with federal, state, and facility regulations. This position involves both leadership and hands-on nursing responsibilities to support our residents' health, dignity, and well-being.
Key Responsibilities:
- Supervise CNAs and LVNs during your shift
- Provide direct nursing care, administer medications, and perform treatments
- Develop and update individualized care plans based on patient needs and progress
- Document observations, interventions, and resident responses accurately and in a timely manner
- Participate in assessments, admissions, discharges, and care conferences
- Respond to changes in residents' conditions and coordinate care accordingly
- Ensure compliance with all facility policies, procedures, and state regulations
- Promote a culture of compassion, professionalism, and teamwork
- Active and unencumbered RN license in the state of California
- Graduate of an accredited nursing program
- Current CPR certification
- One year of experience in a skilled nursing or long-term care facility preferred
- Strong assessment, clinical, and communication skills
- Proficient in electronic documentation systems (PCC experience a plus)
- Ability to lift 25+ lbs and assist with patient mobility
- Must be able to stand and walk for extended periods
- Ability to respond quickly and effectively in emergency situations
- Starting wage of $42/hour
