Pharmacist | per diem (Finance)



Under the immediate supervision of the Director, Pharmacy Services, is responsible for providing pharmaceutical care.

Clinical Competency:

• Ability to interpret physician orders and assure most appropriate medication for the disease state, in the correct dosage, dose form and administration time

• Recognizes medication specific differences related to patient age, gender, disease state and other pertinent patient factors

• Fills medication orders accurately

• Understands when to solicit feedback and input from other healthcare workers

• Maintains knowledge of medication through review of available literature and is able to research and make recommendations about specific medications based upon findings.

• Verify and dose chemotherapy regimens

• Competent in the handling of hazardous medications and does so as needed.

Patient Advocacy:

• Utilizes patient information to implement individualized care

• Information used may include understanding developmental, financial and cultural needs of the patient

• Utilize appropriate resources for non-English speaking patients, hearing and sight impaired individuals, and/or healthcare illiteracy

• Goes the extra mile for our patients when providing retail pharmacy services

• Communications with patients about their medications

Quality:

• Understand role in the Department and Hospital quality initiative

• Suggests ways to improve quality of service, product or process

• Provides feedback to physicians, nurses and other caregivers concerning medications and their use

• Utilizes the Incident Reporting System

• Utilizes clinical intervention documentation within the Pharmacy information system

• Leads by example

• Maintains a clean, neat appearance

• Promotes a positive, can-do, attitude

• Actively participates as a team member

• Assumes responsibilities for actions

Organizational Expectations:

• Attendance: Attendance is an essential job function; employee comes to work daily, on time, and completes scheduled shift

Employee Engagement:

• Actively participates in all hospital Service Excellence initiatives and trainings

• Attends and participates in a minimum of 50% department staff meetings and signs off on review of 100% department staff meeting minutes

• Completes all required annual education on or before the due date

• Minimal physical effort required with occasional light lifting to a maximum of 25 pounds; stooping; and pick, pinch, type, or otherwise work primarily with fingers

• Ability to effectively manage considerable mental stress

• Ability to express or exchange ideas by means of the spoken word

• Ability to receive detailed information through oral communication

• Ability to communicate effectively with co-workers and convey information in a clear, concise and accurate manner

• Familiarity with computer and other business machines

Qualifications:

BS Pharmacy or PharmD

Hospital experience preferred

NH Licensed or eligible
