Are you a passionate and patient-centered Physical Therapist looking to make a meaningful impact? Dublin Post Acute is seeking a dedicated PRN Physical Therapist to join our experienced rehab team. Key Responsibilities:
- Evaluate and treat residents in accordance with physician orders and individualized therapy plans
- Develop comprehensive treatment goals and monitor resident progress
- Educate residents, families, and caregivers to support functional gains
- Collaborate with an interdisciplinary team to ensure the best clinical outcomes
- Maintain accurate and timely documentation in accordance with regulatory standards
- Licensed Physical Therapist (PT) in the state of [Insert State]
- Degree from an accredited Physical Therapy program
- CPR certification (or ability to obtain)
- Previous experience in a skilled nursing or long-term care setting preferred
- Strong interpersonal and communication skills
