Social Services Director (Personal Services)

Victorian Post Acute is hiring a Social Services Director!

We are searching for a compassionate and experienced Social Services Director to lead our social services programs. In this full-time, salaried role, you will be a resident advocate, ensuring their emotional and social needs are met with professionalism and care.

Responsibilities:

• Lead the planning and implementation of resident-focused social services.
• Offer emotional support and counseling to residents and their families.
• Manage and coordinate the discharge planning process with the care team and external agencies.
• Schedule and arrange all necessary on-site and off-site resident services.
• Oversee practical needs, including personal shopping and resident belongings management.
• Maintain effective, empathetic communication with all stakeholders.

Qualifications:

• Bachelor's degree in Social Work or a related field (MSW a plus).
• 2+ years of supervised social work experience in a healthcare setting.
• Ability to handle complex, confidential situations.
• Strong organizational and communication skills.
• Proactive and persistent approach to problem-solving.
